Consent Form PDF

A Consent Form should be provided to each client before performing any lash service on them. The consent form must clearly outline the risks and concerns associated with the services you provide. Before beginning an eyelash extension service, the consent form must be read, understood, dated and signed by the client.

Completed consent forms must be kept in a safe but accessible place throughout the duration of your time seeing each client, as it may be required in the future.
